INSTRUCTIONS:

Choose a publication or client name.

Fill in your name and email address.

If sending an ad, fill in the advertiser name and ad size.

Please include any comments that are important to the files being uploaded.

Click on "Choose File" and select the files (stuffed or zipped file are peferred) to be uploaded. You can upload up to 4 files at a time.

Click on "Upload Files."
